Synopsis on Comparison of cryosurgery with submucosal diathermy in management of inferior turbinate hypertrophy

Introduction / Background
Chronic nasal obstruction is a common complaint often result of inferior turbinate enlargement.Its cause is usually allergic rhinitis or non allergic rhinitis. Inferior turbinate hypertrophy prevents transmission of topical steroids and antihistamines to superior nasal cavity. Surgical reduction of hypertrophied turbinates is recommended when medical treatment fails.Various techniques have been described as turbinectomy,submucosal resection,cryosurgery,submucosal diathermy and laser turbinectomy. Submucosal diathermy was popularized in 1989 although reported in 1970.Coagulation current produces tissue necrosis and the ensuing fibrosis causes shrinkage of soft tissues of turbinates. Cryosurgery causes necrosis of treate nasal mucosa with subsequent regeneration in next one month. many surgeons use SMD as treatment of choice because of its relative lack of complications.Cryosurgery provides significant results when proper patient selection and techniques are used. significance of this study is to find out efficacy of cryosurgery and submucosal diathermy in treatment of chronic nasal obstruction and compare the effects of these techniques.
the objective is this study is to compare SMD with cryosurgery in management of hypertrophied inferior turbinates.
Material and Methods
Cryosurgery is better than submucosal diathermy in management of inferior turbinate hypertrophy.
Study Design : Experimental Study
Sample Size : 60 patients
Sampling Method : convenience non probability type
Duration of Study : six months after approval of synopsis
Inclusion Criteria
1. patients having symptoms of nasal obstruction
2. both male and female
3. patints having allergy treatment for six months
4. patients having drug induced hypertrophy
Exclusion Criteria
1. Patients less than twelve years old
2. patints having nasal polypi
3. history of nasal surgery
Data Collection Procedure
Data will be collected using a proforma.
Subjects will be identiied.
Informed consent will be taken
Patients will be evaluated through history examination of nose and pre operative investigations in SMD.
In outpatient department cryosurgery will be done using cryoprobe along the medial wall of inferior turbinate. freezing will be done for sixty seconds.
procedure may be repeated once or twice as required.
Under general anesthesia submucosal diathermy will be done. diathermy needle is introduced submucosally till posterior end of turbinate. needle was withdrawn over a period of ten seconds with current applied. procedure performed twice on each side.
patients were followed after two weeks.
Statistical Methods
Descriptive statistics will be used to calculate mean, standard deviation, and percentiles. Data will be analyzed on SPSS-10 software
